Paying with a check or needing multiple Services

 

Step 1: Determine amount owed

Using the calendar function and choices on the country page(s) or passport pages, you can determine the amount of your check. Use the back arrow on your browser to go back to the necessary country page or contact us for an amount.

Step 2: Print and fill out the Visa/Passport Request form

Click HERE for the form. Fill it out and enclose it with all the other required documents, your check made out to American Visa Service or authorizaton to charge your credit card (VS/MC/Amex) and send the everything to us:

We suggest you ship documents by UPS Next Day Air as all other options generally arrive after 9:30 a.m. AVS is not responsible for deliveries to our office.
