Please use the following guidelines below when completing the online application or we cannot submit it: 

  • Your name must be entered exactly as it appears in your Passport. Your last name must be entered in the "Surname" box and your first and any middle names or initials must be entered in the "Given name" box. These names must be exactly as they appear in your Passport or the application will be rejected.

  • DO NOT use any punctuation in the fields. Put spaces between words normally separated by a comma or hypen. Do not use periods after abbreviations. Failure to follow this may result in delays and/or additional fees as an application will have to be edited. 

  • ALL applications must request MULTIPLE ENTRIES or they will be rejected

  • US Passports for Business visas must request a 1-year or 5-year multiple-entry visa; Though other options are available on the site, India in Chicago will not issue anything other than 1-year and 5-year visas; the business letter and invitation must also match, in terms of length requested

  • US Passports holders requesting tourist visas must request a 5-year, multiple entry since this is all they will issue.

  • You MUST choose "USA-Chicago" as the Mission Office.

  • The application, once completed online, must be printed on 2 separate sheets of paper; it is NOT accepted on a single sheet. See additional notes below.

  • You must sign the application in 2 places: under the space for the photo and at the end of the application. Sign in blue ink.

  • The address entered on the application MUST match the address on the copy of valid ID provided

  • The address on the letterhead of the US business letter MUST match the Employer address on the application and be in Chicago jurisdiction

  • The address on the invitation letterhead must match the address entered for the application

  • All non-US Passport holders MUST provide an itinerary showing travel plans into and out of India

  • All non-US Passports: On the application you can only request a 1-year visa, especially for Business. The Consulate will not process any other duration for non-US passport-holders; you will have to redo the application

  • All applicants younger than 18 years old must have both parents sign their application twice; one signs in the box on page 1 and the other outside the box with "father" and "mother" indicated next to each signature, the signatures on the second page must be notarized, a copy of the parents' IDs and copies of the picture page of the parents' Passports must be supplied as well as a copy of the applicant's Birth Certificate
