 Step 1: Fill out application online

You need to fill out the DS-4085 application online via the State Department website. Please follow the instructions there to create the form. PLEASE NOTE: In the "Where should the passport be mailed?" section of the application you must put:

Step 2: Assemble documents 

Failure to fill in this information may result in delays and/or additional fees. You must have a current passport to use this form; please locate it to fill in the appropriate sections of the application. You must fill in all the information fields, create the form and print it. Sign and date it.

Passport: You must have a valid Passport; the pages are added to this booklet

Authorization Letter: This letter gives American Visa Service permission to submit and pick up your application. Click HERE to print it. We need one letter for each applicant and it must have the original signature of each applicant or the signatures of BOTH PARENTS in the case of a minor - please write "Mother" and/or "Father" next to each signature. Faxes and/or copies not accepted.

Proof or Departure:  If you require your passport in less than 3 weeks, please provide an itinerary on Travel Agency or Airline letterhead or a copy of your tickets. Without proof of departure, the local Agent may not release your documents and instead mail them into the National Passport Center in which case American Visa cannot help you.

DS-4085 Form: This needs to be completely filled out on the State Department website (see Step 1) and printed. The signature must be original. Faxes and/or copies not accepted.

Step 3: Get documents ready to ship

Once you have made the online application, printed it, and signed and dated it, send all the required documents to:

Passport applications will be delayed or possibly denied if the applicant owes back taxes and/or child support. In these cases, American Visa Service will charge additional Service Fees.

Please again note: You must provide a signed Authorization Letter for each applicant. If you require a passport in less than 3 weeks, please provide Proof of Departure.

Step 4: Place your order

Once you assembled all the documents, place your order with American Visa Service.
